What is Care plan?

A care plan is a written record of what help a person needs, when, who provides it and what to do if something changes.

Good plans cover daily routine, medication, food and drink preferences, how the person communicates, what calms them, what upsets them, and who to call. They are updated as the dementia progresses.

Medicare pays for a dedicated cognitive assessment and care planning visit with a clinician, which many families do not know exists.

Why it matters

It is what lets a new caregiver, a hospital or a relative pick things up without the primary carer present.
