What is Private pay?

Private pay means paying for care from the family's own money, rather than through Medicare, Medicaid, insurance or the VA.

Most in-home dementia care in the United States is private pay, because Medicare does not cover long-term custodial care and Medicaid requires spending down assets first. Sources include savings, pensions, home equity, long-term care insurance and family contributions.

The hourly rate is negotiable with independent caregivers and fixed with agencies.

Why it matters

Knowing the monthly figure early, and how long it can be sustained, shapes every other decision.
