The clinicians below are listed as neurologists in the official NPI Registry. This extract does not include separately verified geriatricians or geriatric psychiatrists, so those specialties are not named here. A neurologist listing also does not, by itself, mean the clinician runs a memory-disorders clinic or focuses on Alzheimer’s disease. Always confirm the current practice location, appointment type, and whether the office evaluates cognitive concerns before you travel or share records.
Alzheimer’s disease is the most common cause of dementia, though it is not the only one. New York public-health agencies have published information on the state’s Alzheimer’s disease burden; use those official pages for statewide context rather than treating this neighborhood list as a prevalence study. How to use this directory
Each entry uses the name, street address, specialty, and NPI number published in the clinician’s NPI record. Phone numbers are not included here because they were not part of the verified extract; call the office only after you confirm a current number through the practice, your insurer, or the NPI record itself.
- Treat the address as the NPI-registered location. Some records use a hospital building, administrative office, or apartment-style mailing address rather than a public clinic lobby.
- Verify that the clinician is still in active practice and seeing new adult patients for cognitive evaluation.
- Ask whether a primary-care or emergency-department referral is required.
- Bring a current medication list, prior imaging or lab reports if you have them, and notes on when symptoms began. This page does not tell you what diagnosis you should expect or which tests you need.
- Use the NPI number if an insurer or hospital operator asks you to identify the clinician precisely.
What to ask when you call
A short, specific call usually saves a wasted trip. These questions are logistical, not a script for self-diagnosis:
- Cognitive evaluations: “Do you evaluate adults for memory, language, or thinking changes, or should we request a different neurology clinic?”
- New patients and wait time: “Are you accepting new patients, and what is the typical wait for a first visit?”
- Referral rules: “Do you need a referral from a primary-care clinician, and what records should be faxed in advance?”
- Insurance: “Do you accept this Medicare plan / Medicaid managed-care plan / commercial plan, and is there a facility fee at this address?”
- Who you will see: “Is the first visit with the neurologist named on the NPI record, or with another clinician in the same group?”
- Location: “Is the appointment at the street address on the NPI record, and which building entrance should we use?”
- Visit format: “Is the first appointment in person, and do you offer telehealth follow-up?”
- Companions: “Can a family member join to give collateral history?”
- Language access: “Do you have interpreter services if we need a language other than English?”
- After the visit: “How are results shared, and who coordinates with our primary-care clinician?”
If the office says the neurologist does not see cognitive cases, ask whether another clinician in the same medical group does. Do not use this page, or a phone script, as a substitute for professional medical judgment about symptoms, diagnosis, or treatment.
