How dementia changes daily care needs
The CDC describes dementia as a general term for the impaired ability to remember, think, or make decisions that interferes with doing everyday activities.
When those difficulties grow, a veteran may need hands-on help with bathing, dressing, meals, and safe supervision. Memphis families often begin with companion care or personal care, then add hours or shift toward memory care at home as needs change.
VA Aid and Attendance eligibility basics
VA Aid and Attendance is an increased monthly pension amount for certain wartime veterans and surviving spouses who already qualify for VA pension and who need regular help from another person with everyday activities, are bedridden, are in a nursing home because of mental or physical incapacity, or have very limited eyesight.
The VA also pays a Housebound increase for eligible pension recipients who are substantially confined to their home because of disability. Both increases are explained on the VA Aid and Attendance and Housebound page.
A dementia diagnosis does not, by itself, approve the benefit. The person still has to meet the VA's service, financial, and care-need rules for pension. Families should file through VA.gov or a VA-accredited veterans service officer rather than assuming they qualify.
The extra monthly amount is paid to the veteran or surviving spouse, not to a specific Memphis agency. Households typically apply it toward care they arrange themselves, including daytime help or 24-hour live-in care when that level of support is needed.
Medicare, private pay, and other coverage
Medicare does not take the place of VA Aid and Attendance for ongoing custodial dementia care. Medicare may cover home health services when someone is homebound and needs part-time or intermittent skilled nursing or therapy under a plan of care. That benefit is limited and is not built as round-the-clock dementia supervision.
The National Institute on Aging explains that people pay for long-term care through a mix of personal funds, insurance, Medicaid, and other programs, including benefits connected to military service. In Memphis, that often means keeping a private-pay caregiver in place while a VA claim or a Medicaid application is pending.
Combining VA benefits with TennCare CHOICES
Eligible Memphis veterans can pursue VA pension income and still look at Tennessee Medicaid long-term services through TennCare CHOICES, but the two programs use different rules and should be coordinated so income and assets are reported correctly.

CHOICES is administered by the Division of TennCare and delivered through managed care organizations. Area Agencies on Aging and Disability (AAAD) offices help people apply. The program has three enrollment groups with different services. Group 2, which covers home- and community-based services, has an active waitlist, so applying as early as possible is recommended.
Financial rules are strict. The look-back period is 60 months (5 years). The asset limit is generally $2,000 for a single applicant and $3,000 when both spouses are applying. Eligibility also depends on a Nursing Facility Level of Care review that looks at activities of daily living, instrumental activities of daily living, and cognitive or behavioral factors. Alzheimer's disease and other dementias can be part of that review, but a dementia diagnosis alone does not guarantee the level-of-care designation. Tennessee does not use a separate, lower threshold just for dementia.

Ask the AAAD and a VA-accredited representative how any VA pension, including Aid and Attendance, will be counted before you change how care is paid. While those reviews are underway, families often use private-pay help or respite care so the primary caregiver is not left without backup.
